To hold a slot for the party we will take a depoist at the bottom of this form and the remainder will be due at the spray tan party.
Please book your spray tan 1-2 days before your event or flight and please schedule all beauty appointments BEFORE your spray tan.
Please let us know if you are on any oral/topical acne medications, retinol products or thyroid medications.
(Green Sun Organics LLC) DBA CleanGlow or our artists Tiffany Capasso, Kelly Wasko, Jennie Iodice, Carolyn DaSilva, Megan Lombardi and Katarzyna Adamaitis will not be held responsible for peeling from previous sunburns and or any topical medications or products. We will also not be responsible for any issues due to improper prep or care. No refunds will be given.
PLEASE make sure to read the pre and post tan instructions, this will ensure your tan develops properly and will provide you the education to care for it. We will not be held responsible for lack of care or use of incorrect products during the life of your spray tan. No refunds will be given.

CANCELLATION POLICY- Under 24 hour per person drop out 100% will be charged to the party card holder. Cancellation of entire booking party under 48 hours will have a $85 cancellation fee.
I understand I am responsible for any lost, stained or damaged items such as clothing, jewelry, furniture including walls and floors or any household items or personal property/belongings. I release (Green Sun Organics LLC) DBA CleanGlow and all artist's Tiffany Capasso, Kelly Wasko, Jennie Iodice, Carolyn DaSilva, Megan Lombardi and Katarzyna Adamaitis from any injury incurred before, during and after my spray tan session.
The FDA recommends that you take the following measures to protect against ingestion or inhalation during the sunless session by using: protective eye wear, nose filters, sealing lips with lip balm and protective undergarments.
DHA is considered safe & non toxic by the FDA for external application. I assume full responsibility for any kind of illness, medical condition, allergic reaction or any type of reaction I may have to this formula from the DHA, any ingredients in the solution/seal/powder/adhesives or any additives added into the solution. I give (Green Sun Organics LLC) DBA CleanGlow and all artist's Tiffany Capasso, Kelly Wasko, Jennie Iodice, Carolyn DaSilva, Megan Lombardi and Katarzyna Adamaitis full consent to apply a spray tan application on my skin and anything else this process entails.
Please have chapstick on for your spray tan session. Lotion on your hands and feet and bring sandals over to our spray tan tent.
