Please fill this out prior to your appointment in order to participate in your spray tan.

Have you sprayed with us before?
Please make sure all self tanner is off.
Please let us know if there is a chance you could be pregnant.
Please understand our formula can change at anytime.
PLEASE schedule all beauty appointments 1-2 days before your sunless tan. Mani, pedi, aesthetic procedures, massage and hair appointments.
Please let us know if you are currently using any oral or topical acne medications, thyroid medications, or retinol products.
Please let us know if you have been sunburn in the past 6 months.
IMPORTANT- Cancellation policy. Deposits on all appointments are non-refundable. We totally get it! Things come up; time is valuable. However, we schedule our lives around these bookings, so please be respectful of our time. Canceling last minute hurts our business by not allowing other clients to book. Under 48 hours 100% of the booked service fee will be charged. 48 hr Reschedule fee-$15.00 must be rescheduled within 2 weeks.
The FDA recommends that you take the following measures to protect against ingestion or inhalation during the sunless session by using: protective eye wear, nose filters, sealing lips with lip balm and protective undergarments.
I give CleanGlow full consent to apply a spray tan application on my skin and anything else this process entails while understanding that no refunds will be given. I as the client assume all responsibly and release all artists Tiffany Capasso, Kelly Wasko, Jennie Iodice, Carolyn DaSilva, Megan Lombardi and Katarzyna Adamaitis for any type of illness, allergic, medical or any other type of reaction to any of the ingredients in the spray tan solution/seal/powder or adhesives.
I agree and take responsibility for any injury incurred before, during or after my spray tan appointment on either mobile or salon/studio appointments and any injury or accident occurring outside our properties. I release CleanGlow from any lost items I may take off during my spray tan appointment.
If switched to a mobile appointment for scheduling reasons I release CleanGlow from any and all liability for any damage to my floors and or walls including spray tan solution on furniture, walls and or floors.
Please make sure to read the pre and post tan instructions on our website, this will ensure the tan develops properly and will provide you the eductation to care for it. We will not be held responsible for lack of care or use of incorrect products during the life of your spray tan. No refunds will be given.
Please book 1-2 days before your event or trip.
Please have chapstick on for your session as the DHA can dry the lips.
If you enjoy your experience, gratuity for our artist's are greatly appreciated.
Only persons being sprayed are allowed inside our facilities and studios.
