Please let us know if you are currently using any oral or topical acne medications, thyroid medications, or retinol products.
Please book your spray 1-2 day before the wedding. Please schedule all your beauty appointments before your spray tan.
PLEASE make sure to read the pre and post tan instructions after you submit this form, this will ensure your tan develops properly and will provide you the education to care for it. We will not be held responsible for lack of care or incorrect product use. No refunds will be given.
I give CleanGlow full consent to apply a spray tan application on my skin and anything else this process entails while understanding that no refunds will be given. I release CleanGlow from any and all liability for any transfer of spray tan solution or damage to my wedding gown or bridemaids gown. I also release liability to any overspray or damage to my floors and or walls including spray tan solution on furniture, walls and or floors in my rental, home or hotel. We take extra caution of course and try our best to keep your location clean. I release CleanGlow from any lost items I may take off during my spray tan or any overspray on any household or bridal items.
I understand I am responsible for any lost or stained items such as clothing, jewelry, furniture or any household items and personal property/belongings. I release (Green Sun Organics LLC) DBA CleanGlow from any injury incurred before, during or after my spray tan session.

IMPORTANT- Cancellation policy. Deposits on all appointments are non-refundable. We totally get it! Things come up; time is valuable. However, we schedule our lives around these bookings, so please be respectful of our time. Canceling last minute hurts our business by not allowing other clients to book. Under 48 hours 100% of the booked service fee will be charged. 48 hr Reschedule fee-$15.00 must be rescheduled within 2 weeks.
The FDA recommends you take the following measures-use of lip balm, nose filters, eye wear and protective undergarments. Please understand our solutions can change at any time.
I as the client assume all responsibly and release all artists Tiffany Capasso, Kelly Wasko, Jennie Iodice, Carolyn DaSilva, Megan Lombardi and Katarzyna Adamaitis for any type of illness, allergic, medical or any other type of reaction to any of the ingredients in the spray tan solution/seal/powder or adhesives. I also agree and take responsibility for any injury incurred before, during or after my spray tan appointment.
Additional Fees- Same Day Requests- $20.00/ Under 24 Hours- $10.00/ After Hours (7pm-9am)- $20.00/ Weekend Mornings-$15.00/ Weekend Afternoons $20.00/ Holiday- $20.00/ Late- $1.00 per minute this includes party participants. Additional fees may apply for any inconvenience to the artist's time.

Please wear chapstick during your spray tan as the DHA can be drying to the lips.
