What are your desired results?

Please let us know if you are on any oral or topical acne meds.

Please let us know if there is a chance you could be pregnant.

Please make sure all self tanner is off.

Please understand our formula can change at anytime.

Please let us know if you have been sunburn in the past 6 months.

PLEASE schedule all beauty appointments 1-2 days before your sunless tan. Mani, pedi, aesthetic procedures, massage and hair appointments.

Please follow our pre and post tan instructions to ensure your tan develops properly and lasts its full tan life. We will not be responsible for lack of care/ improper prep or incorrect product use.

CANCELLATION POLICY- All deposits are non refundable. Under 48 hour drop out or no show 100% will be charged. Reschedule under 48 hours $15.00 fee and must be rescheduled within two weeks.

We have the right to refuse to spray anyone who is severely intoxicated or under the influence of narcotics and who we think may cause harm to themselves or the technician. You will still be charged if this happens for the inconvenience to us.

I understand I am responsible for any lost or stained items such as clothing, jewelry, furniture or any personal property/belongings in my home or that I may bring to my spray tan session . I release CleanGlow from any injuries incurred before, during or after my spray tan session.

The FDA recommends that you take the following measures to protect against ingestion or inhalation during the sunless session by using: protective eye wear, nose filters, sealing lips with lip balm and protective undergarments.

DHA is considered safe & non toxic by the FDA for external application. I assume full responsibility for any kind of illness , medical condition, allergic reaction or any type of reaction I may have to this formula from the DHA, any ingredients in the solution/seal/powder or adhesives or any additives added into the solution. I release CleanGlow from any type of medical condition, illness or allergic reaction and understand no refunds will be given.

CleanGlow will not be held liable for any peeling from previous sunburns and or any topical facial medications.

I agree to all of the above and I give CleanGlow full consent to apply a spray tan application on my skin and anything else this process entails.

If you enjoy your experience, gratuity for our artist's are greatly appreciated.

Please have chapstick on for your session as the DHA can dry the lips.

Disclosure: There is no such thing as 100% organic sunless solution due to the DHA not being organic, this is only 8-15% of the product. However, it is derived from sugar cane and beets and is considered safe for external application for use in sunless products.
